Job Description:
We are seeking a qualified Registered Dietitian / Diabetes Case Manager to provide comprehensive nutrition care and diabetes management services. The candidate will play a key role in patient education, dietary planning, and chronic disease management, particularly for diabetic patients.
Key Responsibilities:
- Assess patients’nutritional needs and develop individualized care plans
- Provide diabetes education and case management support
- Monitor patient progress and adjust nutrition plans accordingly
- Collaborate with physicians, nurses, and healthcare teams
- Maintain accurate documentation and patient records
- Conduct nutritional counseling and wellness programs
- Ensure compliance with healthcare regulations and standards
Required Skills & Qualifications:
- Registered Dietitian (RD/RDN) certification
- Experience in diabetes care and case management preferred
- Strong knowledge of medical nutrition therapy
- Excellent communication and counseling skills
- Ability to work in a multidisciplinary healthcare environment
- Strong organizational and documentation skills
Education Requirements:
- Bachelor’s or Master’s degree in Dietetics, Nutrition, or related field
- Valid Registered Dietitian license/certification
