Job Description
Position: Nurse Practitioner- Hospice
Location: Fresno, CA 93711
Duration: 13 weeks
Shift: 8 Hour Days
State License Details: MUST HAVE CA LICENSE required
REQUIREMENTS:
- Must have Hospice experience
- Palliative NP needed for 1-2 days/week WITH PEDS experience
- This will be based out of the Fresno location. The NP will assist the Medical Director with face-to-face evals who require recertification on Medicare. Will communicate with the CM when any patient requires additional follow up. May round in the inpatient unit. May provide home visits to SNF, care facilities or homes. May educate clinical team members regarding care as needed.
